Civilians continue to bear the brunt of extreme inter-communal violence in Jonglei state in South Sudan, with their resources and lifelines, including hospitals and water supplies, also deliberately targeted, the international medical humanitarian organization Doctors Without Borders/Médecins Sans Frontières (MSF) said today.

Wounded people are still arriving at the MSF hospital in the town of Pibor, three weeks after a violent attack on the town and outlying villages in Pibor County. Many people were injured in the bush, where thousands remain, too afraid to come out of hiding. The hospital was also targeted during the attack.

“We are seeing a cycle of attacks and reprisals throughout this area of northern Jonglei,” said MSF Head of Mission Jose Hulsenbek. “For the civilians in this part of South Sudan, the fear of having to flee their homes or being killed is very real.”

A recurring characteristic of the attacks in Jonglei is their extreme violence. One woman suffering from a gunshot wound and treated by MSF in Pibor said she had fled to the bush with her husband, children, and 15 other family members. After running for eleven hours, they were found by a group of men who shot at them. “We scattered. They shot me in my thigh and my baby was hanging on my back. I tried to hide in the high grass but they found me because my baby was crying. They started beating my daughter until she kept quiet. They left us behind thinking we were dead.” Her son was also treated by MSF for a bullet wound to the chest that remarkably did not kill him.

“After these attacks many women and children are coming to us shot, stabbed, and beaten,” said Colette Gadenne, MSF operations coordinator for South Sudan. “They try to keep safe by hiding in the bush, but it seems that even running away is not enough.”

A deeply worrisome pattern is emerging, where people and their scarce resources are deliberately targeted by all the armed groups involved in inter-communal violence. Hospitals, health clinics, and water sources are all targets, suggesting a tactic of depriving people of life’s basic essentials, precisely when they need them most.

The village of Lekwongole, north of Pibor town, scarcely exists today after coming under attack. All that exists of the MSF clinic there are concrete floors and walls, although medical activities resumed there on January 18.

“The people explained that during the day they dare to come out of hiding to search for food or to seek medical care,” said Karel Janssens, MSF project coordinator. “But at night they return to their hiding places in the bush where they are at risk of contracting malaria or respiratory infections.”

MSF medical teams are now treating serious wound infections, some several weeks old. Since re-launching emergency medical activities in Pibor on January 7, MSF has treated 47 patients with gunshot wounds, among them 16 women and 8 children. An additional 43 patients have been treated for stab wounds, beatings, or wounds sustained while fleeing in the bush. Since January 7, approximately half of MSF’s patient consultations in Pibor have been for malaria; people sleeping in the bush are more vulnerable to contracting the disease.

MSF is extremely concerned for the health and well-being of civilians forced to flee, either from fighting or from fear of an attack. They hide in the bush, with little to no shelter and limited access to food. If they are able to return home, they often only find ashes where their houses once stood.

In the wake of the Pibor attack, MSF has learned that Allan Rumchar, an MSF watchman, and his wife, were killed. Of 156 locally recruited MSF staff members, 25 are still unaccounted for and MSF remains deeply concerned for them.

The violence in Pibor is not isolated. After a January 11 attack on the village of Wek in northern Jonglei State, MSF evacuated 13 patients by air, mostly women and children in need of urgent surgical care at MSF’s hospital in Nasir. That followed an August 2011 attack on the town of Pieri and surrounding villages, during which scores of villagers were killed. In the past six months, 185 people with serious wounds have sought care from MSF teams in Lankien, Pieri, and Yuai.

In a December 2009 report, ‘Facing up to Reality: Health crisis deepens as violence escalates in Southern Sudan’, MSF documented the escalation of inter-communal violence in Jonglei and Upper Nile States, and its increasing impact on civilian populations. MSF treated 392 people wounded by violence that year, and estimated that 86,000 people were displaced. The situation has not improved. In the past six months, MSF has treated more than 250 people wounded in violence in Jonglei State, the majority of whom are women and children.

MSF has been working in South Sudan since 1983, currently in more than a dozen projects in eight states. MSF runs its own medical facilities and supports Ministry of Health facilities in six locations in Jonglei State, providing basic healthcare, therapeutic nutrition, and kala azar and tuberculosis treatment, serving a total population of some 285,000 people. In 2011 the MSF medical facilities in Lekwongole, Pibor, and Pieri were targeted and either destroyed or ransacked during inter-communal violence. MSF condemns the targeting of unarmed civilians and of medical assets by any armed group.

Heading home for the holidays to visit their families after a long break can stir up conflicting feelings for adult children. There’s the nostalgia of the old neighborhood, traditions to relive, the touchstones of memory. But when your parents reach a certain age, going home may also bring up sadness, worry, and apprehension: Are they still okay? What will I do if they’re not?

Currently One Quarter of Adult Children Are Providing Personal and Financial Care For Their Parents*.

According to a MetLife study published earlier this year, the share of adult children “providing personal care and/or financial assistance to a parent has more than tripled over the past 15 years. Currently, a quarter of adult children, mainly baby boomers, provide these types of care to a parent.”

But it’s also true that the physical and/or mental decline of adult parents is a touchy subject. The holidays are rarely the time for taking action for children who see problems with the parents; they’ll likely tell them, perhaps loudly, that it’s none of their business how they’re doing—casting a pall over the holidays for everyone. But concern about how and when adult children’s parents might need their help is still a legitimate one. The best way to begin is to make a quiet, but informed, assessment over the holidays. Three things to keep in mind: Look, Listen, and Pause.

Look

Look means looking for changes in your parents’ ability to carry out simple daily tasks, such as brushing their teeth, taking out the garbage, or putting away dishes. Do they do these tasks with more difficulty or less regularly? Is old food piling up in the refrigerator? Do they walk across a room freely, or are they touching furniture and walls to navigate? If so, they may be having balance problems. Observe how many medications your parents are taking and whether they are taking them in an organized way and according to directions. Medication mix-ups are an increasingly common cause of sudden changes in aging parents’ conditions, especially in this era of pervasive, nonstop medication marketing. Taking the wrong medication at the wrong time in the wrong dosage can lead to multiple problems that quickly snowball. When clients describe changes in a parent’s behavior, memory, speech patterns, or balance, our first suspect is a medication mix-up. Fortunately, when identified, the problem can be solved in a few days.

Listen

Listen means truly listening to your parents as you converse—not interrogating them because you’re worried, or imposing your own expectations. Listen for vague phrases or clichés that signal acceptance or resignation and that are repeated regardless of topic—terms such as “you know, same as ever,” or “can’t complain, I guess.” Do they use these phrases when you ask about a specific activity such as grocery shopping? Can they tell you what a specific medication is for, and who prescribed it? Sometimes people with diminishing mental faculties use this verbal technique, called masking, to hide their increasing confusion. If they are confused, that’s a problem you need to know about—although again, the holidays aren’t the best time to confront it.

To drive or not to drive—that is the most frequently mentioned concern when adult children go home. If it is clear that a parent poses a driving danger you’ll want to deal with that situation without delay—but with a clear understanding that these are among the fiercest of intergenerational family battles. If you don’t want to be the bad guy who takes away the keys—someone is going to have to be—try to find third-party help: their doctor, a family friend, or even the DMV and local police.

Pause

Pause means to give yourself a break and don’t panic if what you see at home alarms you. Resist being overwhelmed. Pause and have a serious talk with yourself or your spouse about the importance of being present, in the moment, and enjoying the holiday. Remind yourself that when you get home, you’ll begin to carve out a long-term plan, with your brothers and sisters. If the problems appear to be too many and too complex to handle yourself, consider seeking help from a reputable, experienced geriatric-care manager or consultant, who will bring objectivity and specialized knowledge into the picture.

Client-Centered Care Management

For the past fourteen years, Aging Solutions has been providing Client-Centered Care Management to elderly and disabled adults. That philosophy means maximizing the integration of community-based services at the local level. This minimizes the need to use the already overburdened health care system as the default resource in a client’s life.

The Client Centered Care Management model is a community-based approach that maximizes the use of all existing community resources while minimizing unnecessary utilization of the health care system. This model is one that is increasingly being recognized in the academic literature as valid, and of benefit to both health organizations and individuals.

Each client in the Client Centered Care Management model has a complex care team consisting of (1) a care manager who provides an onsite assessment and who interacts directly with the patient’s doctors to clarify care issues and troubleshoot in support of the doctor’s treatment plan; (2) a non-clinical care coordinator, in charge of team activities and acting as a second liaison with the medical team; and (3) the client’s physician, who has ultimate authority over medical treatment. The ASI team does not itself provide medical treatment, therapies, or home care, or authorize or deny any medical services.

In many cases ASI acquires a new client during a crisis involving a significant change his or her health care or mental capacity. Once the client is situated securely and stabilized, he or she will experience a significant improvement in their quality of life under our coordinated care approach. Our approach is not only more salutary, it is usually more cost-effective for both the client and family and the local health-care system.

Well-executed Client-Centered Care Management organizes client care sufficiently to avoid unnecessary hospitalizations and emergency department (ED) visits. Client education will assist the client in making better life decisions in conjunction with their health care providers and support system so that utilization can be sharply reduced while quality of life is enhanced.

LHN expects Special Orders and New Marketing Associate orders for products and materials to continue with shipment by the Tampa Global Fulfillment Center. The FBA program will handle customer orders, as well as specific Nutrition Subscription monthly auto-shipments.

As the region’s largest provider of reproductive health care, non-profit Planned Parenthood of Northeast Ohio (PPNEO) echoes the results of a new survey conducted by the Guttmacher Institute confirming the benefits of oral contraceptive pills (OCPs, birth control pills) beyond preventing pregnancy. The new report “Beyond Birth Control,” used data from the federal National Survey of Family Growth to show how women use OCPs to control and manage a wide range of health problems in addition to preventing pregnancy. In fact, 1.5 million women take OCPs solely for non-contraceptive reasons.

The study revealed that after pregnancy prevention (86%), the most common reasons women use the pill include reducing cramps or menstrual pain (31%); menstrual regulation, which for some women may help prevent migraines and other painful “side effects” of menstruation (28%); treatment of acne (14%); and treatment of endometriosis (4%). Additionally, it found that some 762,000 women who have never had sex use the pill, and they do so almost exclusively (99%) for non-contraceptive reasons.

“We know that oral contraceptives are essential health care. This study reinforces that there are other important health reasons why they should be readily available to the millions of women who rely on them each year,” said PPNEO CEO Tara Broderick.

These expanded uses for oral contraceptive pills are mirrored by Americans’ support for insurance coverage of them. According to a recent Thomson Reuters-NPR Health poll, 77 percent of Americans believe that private medical insurance should provide no-cost birth control and 74 percent believe that government-sponsored plans should do the same.

In addition, to oral contraceptive pills, PPNEO offers clients the following birth control methods: birth control implant (Implanon), birth control patch (Ortho Evra), birth control shot, birth control vaginal ring (NuvaRing), cervical cap (FemCap), diaphragm, condom, fertility awareness method and IUD (Mirena and ParaGard).
